We’ll check in regularly, help you stay on top of medications and appointments, and make sure nothing falls through the cracks. You’re not in this alone—we’ll walk beside you every step of the way.
Living with a chronic condition doesn’t mean you have to manage it alone. Our Chronic Care Management program provides personalized, monthly support for patients with two or more chronic conditions. From regular check-ins and care coordination to medication management and goal setting, we work with you every step of the way to improve your quality of life and keep you out of the hospital.
What is chronic care management?
Chronic Care Management (CCM) is built for people who are juggling two or more long-term health issues—and feeling like they're doing it alone. Whether it's anxiety and ADHD, high blood pressure and diabetes, or any mix of physical and mental health conditions, CCM gives you monthly support to stay on track. It’s not just about checking boxes—it’s about making sure your care is working for you, not the other way around.
What happens during chronic care management?
Every month, we check in to see how you’re doing—physically, mentally, emotionally. We track your symptoms, help manage meds, coordinate labs or referrals, and talk about what’s actually helping and what’s not. You’ll have a real person (hi, it’s us!) helping you stay organized, focused, and not so overwhelmed. We become your go-to team between visits, so nothing falls through the cracks.
When should I consider CCM?
If you’re dealing with multiple chronic conditions and feeling like managing it all is a full-time job, CCM is probably for you. It’s covered by most insurance plans and gives you access to extra support between appointments—because your health doesn’t take a break, and neither do we.
